CARECONTACT
 



CareContact is a customized integrated medical management system that provides a set of communication and documentation tools to create a control point for member health management. Information that is generated from health risk assessment tools, claims, pharmacy and lab data are integrated into CareContact. This system allows us to manage members with varying degrees of health awareness and health needs within the same database. The structure is built around the identification of interventions for health, wellness and chronic disease. Gaps or noncompliance to these interventions are monitored, enabling us to assist members in a timely manner.

CareContact’s ongoing customization allows us to address our member needs in different markets. The ability to set access levels with individual passwords provides each market with the level of CareContact that best addresses their needs. Members have access to health and wellness tools, medical library, a personal evaluation system and disease specific report cards to assist in the management of certain chronic conditions. Case managers have user-friendly tools at their desktops to assist them with resources, member information and enable them to set reminders for interventions needed to assist a member.

Whatever our goals, from preventative screening measures to disease management, CareContact provides the documentation management tool and reporting capabilities to better manage our membership’s health.
