SelectCare of Oklahoma offers a Special Needs Medicare Advantage plan known as Tribute that is designed to meet the needs of people living in skilled/long term care facilities across 10 counties in South Central Oklahoma. By providing a specially designed product to service this population, SelectCare of Oklahoma will address the special needs of Medicare beneficiaries residing in institutions or who meet the CMS (Centers for Medicare and Medicaid Services) definition of “institutionalized.” The Tribute plan gives Medicare eligible institutional beneficiaries the opportunity to take advantage of managed care coordination through an HMO.
Tribute makes it easier for its members to access benefits and receive the care they need by providing a mid-level practitioner (MLP), a nurse practitioner or physician assistant, who works in a team environment with the nursing facility staff and primary care physician (PCP) to provide ongoing care to institutional residents. MLPs provide a comprehensive level of care coordination between the beneficiaries’ family and physicians, as well as arrange preventive care to help members stay healthy and living as independently as possible.
